Important information

Please look over the html fragment very carefully. You will need to look for _INSERT YOUR E-MAIL HERE_ , simply delete that and type in your e-mail.
Next look for _INSERT YOUR NAME HERE_, again, just delete that and add your name.
Now look for _INSERT YOUR SITE ID HERE_, once again, delete that and type in your site ID.
